I agree that I:

Am 18 years or older
Have not had a hypersensitivity reaction to ofloxacin, other quinolones (i.e., ciprofloxacin, moxifloxacin), or any component of its formulation
Am not pregnant or breastfeeding
Am using this medication as indicated to treat an ear infection (otitis media or otitis externa)
Will use this medication for the appropriate duration of therapy of m7 days and understand that prolonged use may result in fungal or bacterial superinfection
Will discontinue the medication if I develop a skin rash
Understand that although rare after otic use, tendon rupture/inflammation has been reported with use of ofloxacin and I will discontinue at first sign of inflammation
Understand this is for use in the ears only, avoiding contact with the eyes and mouth
understand this is not a comprehensive list of precautions, risks, side effects, or details about taking this medication
understand that only major drug interactions are listed and have not been reviewed against the other medications, supplements, or herbal products I may be taking unless also filled by TelyRx
