About NAPIS Forms

National Aging Program Information System (NAPIS) forms are required by the federal government and are used by LSS Meals to:

  • Track how many meals and people are served.
  • Better understand the needs and situations of the people we serve.
  • Evaluate the effectiveness of our services.

Prior to receiving your first meal, you will be asked to complete a NAPIS form in person with either an LSS Meals employee or volunteer. This will be required annually.

We are committed to protecting your confidential information.
LSS submits this information to governmental agencies to receive funding. No outside agency tracks information about you personally. When we enter the information a member number is generated — this member number is submitted to the federal and state agencies, participants’ names are not included.
