Zusammenfassung
Üblicherweise existieren in der Literatur folgende Definitionen zum Diagnosebegriff:
Fehldiagnose: unrichtige Diagnosestellung bei abgeschlossenem, diagnostischen Entscheidungsprozess, daraus resultierend falsche Behandlung und verschlechterte Prognose des Patienten. Diese klare Definition unterscheidet die Fehldiagnose von Begriff en wie der Arbeitsdiagnose: bewusst vorläufige Diagnosestellung, Absicherung durch Verlaufs- und Zusatzuntersuchungen erforderlich oder der Differentialdiagnose: verschiedene in Betracht zu ziehende Erkrankungen, bewusst jedoch keine definitive diagnostische Entscheidung. Darüber hinaus ist die Fehldiagnose abzugrenzen von der falschen Diagnose (false positive diagnosis), bei der klinisch eine Diagnose gestellt wurde, die sich pathologisch-anatomisch nicht bestätigte, ohne dass dies für die Prognose des Patienten bedeutsam ist und dem Begriff der nicht gestellten Diagnose (false negative diagnosis), bei der es sich um klinisch unerkannte, für die Prognose des Patienten irrelevante Befunde und Erkrankungen handelt, die pathologisch-anatomisch nachgewiesen wurden.
Trotz enormer Fortschritte in den diagnostischen Möglichkeiten — insbesondere im apparativ-technischen Bereich — sprechen die wenigen Analysen, die es im Schrifttum der letzten Jahre und Jahrzehnte zum Thema Fehldiagnosen gibt, mit einer Ausnahme dafür, dass die Fehldiagnosenhäufigkeit in Großkrankenhäusern und Universitätskliniken Mitteleuropas und Nordamerikas unverändert bei etwa 10 % liegt. Lediglich eine im Jahre 2000 veröffentlichte Untersuchung aus dem Züricher Universitätsklinikum geht von einem Rückgang der Fehldiagnosenrate aus (Sonderegger-Iseli, 2000 [89], wobei sich die Definition der Fehldiagnose in dieser Arbeit von der o.g. unterschied. Die am häufigsten beobachteten Fehldiagnosen sind Lungenembolien, Myokardinfarkte, Malignome und Infektionen, insbesondere Pneumonien. Die wichtigsten Ursachen von Fehldiagnosen dürften die Vernachlässigung der klassischen klinischen Untersuchungsmethoden, die inadäquate Gewichtung und Fehlinterpretation von Befunden apparativ-technischer Diagnoseverfahren und der erhebliche Rückgang der Obduktionsfrequenz in unseren Krankenhäusern sein (Verlust des „errando discimus“). Aus dem Letztgenannten ergeben sich Empfehlungen zur Vermeidung von Fehldiagnosen; in diesem Zusammenhang ist insbesondere die Bedeutung der sorgfältigen Anamneseerhebung sowie der körperlichen Untersuchung zu nennen, denen nach wie vor der höchste Stellenwert im Hinblick auf die definitive Diagnosefindung beigemessen wird.
